ü Must be Female , age between 23 to 45 years old
ü With Valid Passport of at least 18 months
ü Confirmed
Fit to work from the accredited Medical Centre by the Malaysian representative office in
Philippines
ü Presently residing in the Philippines
ü enter
Malaysia via the Visa With Reference (VDR) issued by the Malaysian
representative office in the Philippines
ü Certified
PASS for IMMIGRATION SECURITY CLEARANCE (ISC) in the
Philippines
ü Completed
training course under TESDA
3. Employer Eligibility Requirements:
v Application can be submitted through maid agency in Malaysia with valid
approved license from JTKSM and Accredited with Philippine Overseas Employment
Administration (POEA
v Employers
who have been declared bankrupt are not eligible to apply.
v In
order to apply for second Foreign Domestic Helper, the employer must:
1. have
a valid reason
2. Have
income under the conditions stipulated by the Immigration Department of
Malaysia.
4. Selection
of Philippine Domestic Helper Biodata
5. Arrangement
of Skype Interview or Phone Interview with the selected Biodata Confirmation of
selected biodata , considered BOOKED only upon settlement of Deposit due amount

The maid agency will request a copy of the employer IC/Passport and complete address for the issuance of
Invoice
6. Settlement
of deposit payment
·
Issuance of paid Receipt to the employer and the
Philippine Domestic helper biodata will remove from the website as confirmed
book.
7. The
agency representatives will coordinate to the employer for the signing of
application forms and completing the required documents stipulated by the Immigration
Department of Malaysia before the submission of VDRApplications for the
approval of foreign domestic helpers can be made at all State Immigration
Offices in accordance with the employer’s address or
can be handled by employment agencies registered with the Department
a) Documents
required for the application of a Visa With Reference (VDR) for Foreign
Domestic Helpers:
5. Employment Agreement for
Foreign Domestic Helpers which has been stamped with a RM10.00 revenue stamp (2
sets of originals and copies)
6. Copy
of Foreign Domestic Helper’s passport
7. Medical
Report from the country of origin which has been certified by a clinic
appointed by the Ministry of Health Malaysia
8. Immigration
Security Clearance (ISC) certificate
9. Required
documents of employer by the Immigration Department of Malaysia.
8. Upon
the VDR approval, the agency will arrange to pay the levy according to the type
of the nationality of the Foreign Domestic Helper.
During
the application of Foreign Domestic Helper to arrive in Malaysia with
unforeseen circumstances and the application of Foreign Domestic Helper
candidate is unable to arrive in Malaysia, the following situation can proceed
with new application by Replacement of Foreign Domestic Helper (FDH)
a. The
employer may submit an application for a change of or replacement of the Foreign
Domestic Helper that has been approved for Visa with Reference due the worker
did not enter the country after the approval of the VDR is issued.
In
order to apply for a replacement, the employer should submit the required
documents:
1. Employer’s
application letter ( with the new selected Philippine Domestic Helper biodata)
2. Letter
of approval for Visa With Reference
3. the
original payment receipt
4. letter
of visa cancellation from the representative office in the employee’s country
of origin (if any)
9. Documents
required for the Application of
Employment Contract at the Philippine Consulate, Malaysia (POEA)
1. Standard
Employment Contract Form ; signed and notarized
2. Original
VDR approval letter ( Visa with reference )
3. Original
Levy receipt
4. Copy
of Foreign Domestic Helper’s passport
5. Copy
of the Employer’s IC/ passport
6. Information
sheet
7. Affidavit
of Undertaking; signed and notarized
10. Upon
the released of Verified and Authenticated Employment Contract of the Employer
and the Foreign Domestic Helper from the Philippine Consulate, Malaysia.
Ø
The employment agency will arrange to send all
original documents to the recruitment agency counterpart for the deployment
processing of your selected Philippine Domestic Helper.
11. The
recruitment counterpart agency and Foreign Domestic Helper has received the
Approval letter for Visa with Reference to get a visa at the Malaysian
Representative Office and the Authenticated Employment Contract to process the
Issuance of Overseas Employment Certificate (OEC) from POEA office in the
Philippines prior to entry into Malaysia.
12. After
getting the visa and OEC the Foreign Domestic Helper can enter Malaysia through
any permitted entrance. Immigration officers at the gates of entry will provide
a Special Pass for 30 days so that the employer can report the presence of the
Foreign Domestic Helper at the Immigration office that approved the VDR. This
Immigration office can be the State Immigration Office or Immigration
Headquarters
13. Arrival
of your Philippine Domestic Helper in Malaysia
Ø
Arrangement to fetch your Philippine Domestic
Helper in the employment agency, together with the settlement of your final
payment due amount.
Ø
The employer or employment agency is required to
bring the FDH for a medical check up at any clinic which is registered with
FOMEMA Pte. Ltd. (within a month).
Ø
Any Foreign Domestic Helper who fail the FOMEMA Medical examination must
be sent back to their home countries immediately
a.
The employer may submit an application for a
change of or replacement of the Foreign Domestic Helper that has been approved
for Visa with Reference due the worker has entered the country but is NOT
medically fit by FOMEMA Pte. Ltd.
In
order to apply for a replacement, the employer should submit the required
documents:
1. Employer’s
application letter (with the new selected Philippine Domestic helper biodata)
2. Letter
of Approval for Visa With Reference
3. Check
Out Memo
4. the
original payment receipt
Ø The
Foreign Domestic Helper who has passed the medical examination and is certified
healthy by FOMEMA Pte. Ltd. will be issued with the Visitor’s Pass Sticker
(Temporary Employment) in accordance with the approved period of VDR in the
approval letter.
Ø
And obtain the PL (KS) sticker from the State
Immigration Department which will be issued within 1 month from the date of
arrival.
Ø
The
employer shall provide the Philippine Domestic Helper with personal life,
accident, medical and repatriation insurance from a reputable insurance company
in Malaysia.
Ø
Once the Philippine Domestic Helper has received
the Visitor’s Pass Sticker (Temporary Employment), she is allowed to work until
the deadline stated on the sticker. PL (KS) concerned.
14. Extension
of the Visitor's Pass/Temporary Employment (PL/KS) (Renewal of Visa)
Ø Applications
for the extension of a Philippine Domestic helper PL (KS) can be made at the
Immigration Office of the State within three months of the PL (KS)‘s expiry
date.
Ø
Employers must ensure that the Philippine
Domestic Helper undergoes a medical examination at any clinic that is
registered with FOMEMA Pte. Ltd. and certified healthy before any application
for second and third year extension of service is submitted to the Immigration
Department of Malaysia.
Ø All
matters pertaining to Philippine Domestic Helper extension of service [PL (KS)]
must be submitted by the employer unless the renewal of the FDH can be handled
by employment agencies registered with the Department.
Ø Incomplete
application or application that is not in order will be rejected.
Ø Payment
for the extension includes the levy, visa fees according to the type of visa
and the nationality of the FDH, the payment process and fees for the PL (KS).
Ø
Documents required:
1. Copy
of Worker's passport with a validity period of not less than 12 months.
2. The
payment in the form of bank draft or money order should be made payable to the
Director General of Immigration, Malaysia or the Director of Immigration
Affairs (where payment is made). Name and address of the employer must be written
on the back of the bank draft
Notice:
Make sure the FDH has passed the medical examination by FOMEMA Pte. Ltd. for
second and third year extension of service only.
16. Check Out Memo Application
a. The
Check Out Memo is issued to foreign workers/ foreign domestic helpers to
facilitate their return to their country of origin.
b. In
order to apply for the Check Out Memo, the employer should submit the required
documents:
1. Application
form for Check Out Memo
2. Copy of Original passport of foreign worker.
3. Ticket
for the foreign worker to return to the country of origin.
c. The
employer should ensure that the foreign employee’s returns to their country of
origin via the requested exit point on the date stipulated in the Check out
Memo.
d. If
the employer fails to apply for the Check out Memo, the foreign workers are
still considered to be under his employment and the employer is still
responsible for the foreign workers.
17. How
to Report Foreign Domestic Helper who have absconded
a. The
employer / reporter must be present at the relevant Immigration Office to
submit the following documents so as to cancel the Foreign Domestic Helper’s PL
(KS).
1. Letter
of application to cancel the PL (KS) of the Foreign Domestic Helper who has
absconded.
2. Copy
of employer’s / representative’s identity card
3. Copy
passport of foreign domestic helper who has absconded
4. Foreign
Worker Abscondment Report Form issued by the Immigration
Department of Malaysia
5. Certified
copy of Police Report
b. Foreign
Domestic Helpers who have absconded will be blacklisted.
c. Employers
are required to pay bail on personal bond based on the rates that have been
signed during the application.
