1-1           Married Employers:

·        NRIC copy of the employer and spouse

·        Passport size photo of the employer and the spouse

·        Proof of Income of the employer or combine with the spouse  must be above 5,000 Ringgit per month

·        Proof of  latest Income ( LHDN / EA / EC / PENSION / IRB / IR8 A / EPF STATEMENT and  RECEIPTS

·        Supporting documents ( E-filling statement  and receipts / latest bank statement / employment confirmation and latest 3 months salary slip)

·        Marriage cert

·        Reason of applying a Maid in Malaysia is for:

(1) Taking care of children

v Children birth cert under 15 yrs old

(2) Taking care of spouse

v Original Medical Certificate from Government hospital or Specialist

(3) Taking care of Parent

v Original Medical certificate from Government hospital or Specialist

v IC copy of the stated patient

v Birth cert of the employer

(4) Taking care of Parent In-law

v Original medical certificate from  Government hospital or specialist

v IC copy of the stated patient

v Birth cert of the spouse of employer

(5) Big House

v Photos of the house  ( for landed house : Bungalow / 2 or 3 storey house)

i.       Frontal view of the house (the house number must be visible in the photograph

ii.     Interior of the house

iii.  Street name

v Photos of the house ( for condominium / Pent house)

i.       Frontal view of the door ( the door number must be visible in the photograph)

ii.     Interior of the unit

iii.  Frontal view of the condo ( the condo name must be visible in the photograph

iv.  Street name

(6) Copy of Latest Electricity bill – proof of employer current address

v In case the TNB billing name is different person

i.       IC copy of the stated name on the TNB bill

ii.     If  the TNB bill under the parent /parent in-law name ; must submit a birth cert of the child of the stated person in the TNB bill

iii.  If the TNB bill under the house / condo owner: must submit the copy of latest Tenancy agreement.

Ø Additional documents under 2^nd maid application:

ü Reason for 1^st maid ( take care of children or take care of sick spouse or  Elderly care)

ü 1^st maid copy of passport and latest visa

ü Reason for 2^nd maid (  take care of sick spouse or Elderly care or Big house)

ü Proof of income of the employer or combine with the spouse must be above 10,000 Ringgit per month

1-2 Under Local/Malaysian - Single Employers:

·        NRIC copy of the employer

·        Passport size photo of the employer

·        Proof of Income of the employer must be above 5,000 Ringgit per month

·        Proof of  latest Income ( LHDN / EA / EC / PENSION / IRB / IR8 A / EPF STATEMENT and  RECEIPTS

·        Supporting documents ( E-filling statement  and receipts / latest bank statement / employment confirmation and latest 3 months salary slip)

·        Affidavit of Singleness ( Surat Berkaun)-  with commission of oath

·        Reason of applying Maid for:

(1) Taking care of children

(2) Taking care of the employer

v Original Medical Certificate from Government hospital or Specialist

(3) Taking care of Parent

v Original Medical certificate from Government hospital or Specialist

·        Passport copy and latest MM2H Visa of the children below 15 yrs of age

·        Copy of the MM2H approval letter

·        Copy of SNP

·        Copy of the latest Electricity bill

3-    Employers under Permanent Resident (PR):

·        Copy of  Red IC of the Employer and the spouse

·        Passport size photo of the employer and the spouse

·        Copy of marriage cert ( in English Translation)

·        Copy of Proof of Income

·        Copy of Red IC and Children birth cert below 15 yrs of age

·        Copy of latest Electricity bill

·        Copy of SNP

4-    Employers under Resident Pass (RP):

·        Passport copy and latest visa of the employer and spouse

·        Passport size photo of the employer and the spouse

·        Copy of marriage cert ( in English translation)

·        Passport copy and latest visa of the children below 15 yrs of age

·        Copy of Tenancy agreement or SNP

5-    Employers under Expatriate ( MIDA/MDEC/Putrajaya):

·        Passport copy and latest visa of the employer and spouse

·        Passport size photo of the employer and the spouse

·        Copy of Marriage cert ( in English translation)

·        Passport copy and latest visa of the children below 15 yrs of age

·        Latest copy of TNB bill

·        Confirmation letter from the company of the employer address to immigration office:
